Fill the forms out to the best of your ability. If your visit isn’t accident related you can leave that section blank. Be sure to bring in any paperwork related to your condition or accident with your to your appointment (Police Report, MRIs, Previous X-rays, etc.). Also if you are using health insurance be sure to bring your insurance card with you to your appointment so we have all the correct information to submit your claims.
Pediatric New Patient Paperwork :
(13 years old and younger)
These forms will need to be printed at home, filled out, and brought into the office. If you don’t have access to a printer, don’t worry! Just show up 15 minutes prior to your appointment time to fill the forms out in the office.
